The health form is available through the website Spain Travel Health or from the app SPAIN TRAVEL HEALTH-SpTH (available on Google Play and the AppStore).
The form is divided into 5 parts:
- Personal information
- Travel information
- Travel background
- Health questionnaire
- Certificate
Therefore, you will need to have information such as seat number, details of your vaccine doses (if you travel vaccinated), etc.
